Please read, complete, and sign the waiver below before your ride.
Cycling — including riding electric bicycles — involves inherent risks such as collision, falls, surface hazards, equipment variation, and serious injury or death. E-bikes (Ohio ORC §4511.522 Class 1, 2, or 3) are heavier and faster than standard bikes. I understand these risks and voluntarily accept them.
I agree to: wear a CPSC/ANSI-approved helmet at all times (required by shop policy); ride only on designated roads or paths as directed by staff; start at low assist levels and increase speed only as I grow comfortable; and stop riding immediately if I feel unsafe or notice any mechanical issue.
I certify that I am in good physical health, have no medical condition that would endanger myself or others, and am capable of safely operating the bicycle provided.
For myself and my heirs, executors, and successors, I hereby waive, release, and covenant not to sue Sixth City Cycles, its owners, employees, and agents ("Releasees") from all liability, claims, losses, or damages arising from my participation in this test ride, including those resulting from the ordinary negligence of the Releasees. I further agree to indemnify and hold harmless the Releasees — including any attorney's fees — from any claim brought contrary to this agreement. This release does not apply to wanton or willful misconduct, which is non-waivable under Ohio law.
This agreement is governed by the laws of the State of Ohio. If any provision is held invalid, the remaining terms continue in full force and effect.
By signing, I confirm I am 18 or older, have read and fully understand this agreement, and sign it freely and voluntarily as a complete release of liability to the fullest extent permitted by Ohio law.
